1. INTRODUCTION The use of alcohol, illicit drugs or other psychoactive substances, is increasingly a problem confronting health authorities in both the North and the South. As more information is available on patterns of use, so it has become evident that the chronic and acute abuse of these substances is associated with a range of physical and psychosocial manifestations. To date, the emphasis in research has been predominantly on the health problems of the individual substance user, and on ways in which prevention, treatment and service delivery could be better structured to meet the person's needs. Within existing medical and psychological frameworks, the substance user is largely seen as being at risk of a multitude of health and associated behavioural disorders. The user is typically portrayed as a psychologically or socially marginalized individual, who is difficult to reach and even more difficult to work with. As a patient requiring treatment and counselling, the individual is, at times, perceived as outside the everyday social networks to which he or she is part.
